A fast-growing healthcare organization is launching a brand-new primary care model and is seeking a high-performing Nurse Practitioner to serve as the first hire in the Norfolk/Chesapeake market. About the Role

• Position: Primary Care Nurse Practitioner

• Model: New primary care initiative focused on chronic disease management for a complex adult population

• Setting:

  • Primarily in-person clinic-based care across several local sites

  • Some flexibility for remote charting (half days as needed)

  • Field-based presence across multiple clinics

• This is not a traditional dialysis or specialty NP role—it is true primary care delivery within a new, high-visibility model.

What We’re Looking For

Required Experience:

• Minimum 5 years of NP experience in primary care, internal medicine, gerontology, or hospitalist medicine

• Proven ability to manage patients with chronic, complex comorbidities

Compensation & Incentives

• Salary: $130,000 - $145,000/year base 

• Performance Bonus: Up to 60% of base salary
