Primary Care Nurse Practitioner Job Details

  • Position Type: Locum tenens Nurse Practitioner (contract, temp-to-perm potential)
  • Location: Multiple clinic sites in Norfolk, Great Bridge, Chesapeake, Leigh, Midtowne, Kempsville, Southern Tide, and Little Creek, VA
  • Duration: 16 weeks, with strong potential for extension or permanent placement
  • Shifts per Week: 5
  • Shift Duration: 8-hour days (clinics open 6:30am–7:00pm; flexible scheduling)
  • Hours: Approximately 40 hours/week; no nights, no weekends
  • Healthcare Setting: On-site, clinic-based with some fieldwork and occasional remote charting

Nurse Practitioner Qualifications

  • Minimum 3 years of Nurse Practitioner experience in primary care, family medicine, or internal medicine (5 years preferred)
  • Active and unrestricted Nurse Practitioner license in Virginia with autonomous practice authority
  • Experience managing chronic comorbid conditions; nephrology or dialysis experience is a plus (including RN experience)
  • Entrepreneurial mindset and comfort with program development and healthcare innovation
  • Proven ability to build strong patient relationships and deliver continuity of care
  • Leadership potential and excitement for growth in healthcare jobs
  • Not suitable for acute care, urgent care, surgical, ICU, or pediatrics backgrounds

Primary Care Responsibilities

  • Provide direct primary care to ESKD patients at multiple clinic locations in the Norfolk, VA area
  • Manage a panel of patients, delivering comprehensive chronic disease management and preventive care
  • Collaborate with nephrologists, dialysis teams, and clinical support staff for holistic patient care
  • Establish and nurture long-term patient-provider relationships as the primary Nurse Practitioner for scheduled visits
  • Exercise autonomy in ordering and interpreting labs, managing medications, and coordinating care
  • Contribute to the development and scaling of a new care delivery model, sharing feedback with healthcare leadership
  • Adapt to evolving patient volumes and program needs, demonstrating flexibility and initiative
