Advanced Practice Provider (NP or PA) Outpatient Palliative Care New

NJ Palliative Care Solutions is seeking a compassionate and motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our outpatient palliative care team in Union County, NJ. This role focuses on supporting patients with serious illnesses in the community setting, ensuring comfort, dignity, and quality of life outside the hospital.

Key Responsibilities:

• Provide outpatient palliative care visits, including patient assessments, symptom management, and individualized care planning.
• Collaborate with physicians, social workers, and other care providers to deliver comprehensive patient-centered care.
• Educate and counsel patients and families on disease progression, treatment options, and advance care planning.
• Maintain accurate clinical documentation and communicate effectively with referring providers and care teams.
• Participate in quality improvement initiatives and community outreach as needed.
  
  

Qualifications:

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in New Jersey.
• Previous experience in palliative care, hospice, geriatrics, oncology, or internal medicine preferred.
• Strong communication and interpersonal skills with the ability to build trusting relationships with patients and families.
• Comfortable working in outpatient and community-based settings, including clinics and patient homes.
