Advanced Practice Provider (NP or PA) Palliative Care New

NJ Palliative Care Solutions, in partnership with RWJ University Hospital Somerset, is seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our inpatient palliative care team. This position offers the opportunity to provide meaningful support to patients and families while working within a respected community hospital.

Key Responsibilities:

• Conduct inpatient palliative care consultations, including comprehensive patient assessments and symptom management.
• Collaborate with the interdisciplinary team—physicians, nurses, social workers, chaplains, and other specialists—to coordinate patient-centered care.
• Facilitate family meetings and provide education, counseling, and emotional support.
• Participate in care planning discussions and quality improvement initiatives within the hospital.
• Ensure timely and accurate documentation of all clinical encounters.
  
  

Qualifications:

• Current NJ license as a Nurse Practitioner (NP) or Physician Assistant (PA).
• Prior experience in palliative care, hospice, geriatrics, or internal medicine preferred.
• Strong interpersonal and communication skills.
• Ability to work collaboratively in both patient-facing and interdisciplinary team settings.
