Who You Are:

✅ Licensed Nurse Practitioner in New York

✅ Master’s or Doctoral degree in Nursing (AGNP or GNP preferred)

✅ Board Certified (or eligible)

✅ Eager to work in geriatrics – new grads welcome!

✅ Skilled in EHRs, chronic disease management, and collaborative care

✅ Compassionate, communicative, and culturally sensitive

What You’ll Do:

• Provide comprehensive exams and manage chronic/acute conditions
• Develop and implement personalized care plans
• Order and interpret diagnostic tests
• Prescribe medications and track patient progress
• Educate patients and families on health management
• Collaborate with physicians, therapists, and nursing staff
