Job Description
PURPOSE OF POSITION:
Under the general administration of the Department of Medicine and in the collaborative relationship with Nursing, coordinates, manages, and provides care to selected patients within protocols mutually agreed upon by the medical and nursing personnel, and within the scope of practice as defined by the New York State Law.

Job Description
PURPOSE OF POSITION:
Under the general administration of the Department of Medicine and in the collaborative relationship with Nursing, coordinates, manages, and provides care to selected patients within protocols mutually agreed upon by the medical and nursing personnel, and within the scope of practice as defined by the New York State Law.
Duties And Responsibilities
- Maintains clinical privileges in the Department of Medicine.
- Prescribe medications and order tests in accordance with the Delineation of Privilege on file with the Medical Board.
- Documents in the chart all care rendered to patients and all collaboration with the physician.
- At the time of the visit to discuss with the collaborating physician; acutely ill patients, patients who are not following the expected clinical course, and complicated patients.
- Discuss with the collaborating physician before ordering requests for sub-specialty consultation, invasive procedures, radiology studies involving the use of contrast, nuclear medicine studies, and MRI.
- Participate in the PI Process of the Department of Medicine, including the use of tracking and trending files, and random and focused chart reviews.
- Obtains pertinent health history which includes physical and psychosocial history.
- Triages and establishes priorities appropriately.
- Performs comprehensive physical examination.
- Requests appropriate screening and diagnostic tests.
- Gather additional relevant patient information.
- Interprets and evaluates data gathered.
- Develop a plan of health maintenance for the patient in coordination with the physician and other team members.
- Designs and implements an individualized patient care plan in accordance with established protocols.
- Makes appropriate referrals to other health care providers and socialists after consultation with the collaboration physician.
- Identifies health education needs and implements plans to prevent and/or improve chronic illness disability and promote safety and wellbeing.
- Documents patient’s medical history, clinical findings, and therapeutic plan in the medical record.
- In accordance with protocols or with the concurrence of the supervising physician, prescribes medications and immunizations.
- Ensure a safe and secure environment free of hazards to reduce the risk of injury to all customers (patients, visitors, and staff.)
- Demonstrates responsibility to co-workers, unit, and institution.
- Effectively utilizes the electronic as well as the paper medical record.
- Participates in Performance Improvement activities.
- Completes other related nursing activities as assigned.
- A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
- Current certificate as a Nurse Practitioner issued by the NYSED; and
- Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
